The use of e-learning, electronic teaching tools and digital assessments have become the norm in universities worldwide.
Despite the positives of these changes, there are still many challenges associated with the implementation of online examinations. These include student perceptions, student performance, anxiety, cheating, staff perceptions, authentication and security, interface design, and technology issues.
A systematic review was carried out to identify the existing literature on online examinations and examine their benefits and challenges in comparison to traditional invigilated exams. The review identified key themes that could be addressed in a more comprehensive way, including: the underlying needs for online examinations (e.g. accessibility, ability to distinguish a true pass from a true fail, secure, minimize opportunities for cheating, accurately authenticate students, reduce marking time), and the role of the administrator in these processes.
Based on the aforementioned themes, we developed and tested an online OSCE system for high-stakes examinations in the context of the COVID-19 pandemic. The system was designed with the aims of being feasible, cost-effective, acceptable and valid.
The system was implemented in the form of an online time management website, which was accessed by students and staff to track their progress through each station, monitor their movement between stations and record their activity in each station. It was also designed with the aim of providing a clear and easy-to-understand link to their virtual room for all users, as well as an indication when they should start and finish each station.
The online OSCE system was used in three examinations and one short mock OSCE for training purposes. The exams involved Year 3 and Year 4 students, with six history taking and communication stations with examiners and simulated patients (SPs) and four rest stations. The exams were conducted using an OSCE time management website to guide the student and staff through the exam. In addition, a short survey was distributed to the students seeking their opinions on the training and use of the system.
